A letter to the boss

Subject: Medical Leaves.
Date: Today

Dear Sir,

Im writing to with regard to the above.

The purpose of this email is to tell you that I was advised by the orthopedic surgeon of Ampang Puteri Specialist to undergo an ACL (Anterior Crutiate Ligament) reconstruction surgery on my left knee. Early MRI scan showed that the ACL tear of one of my ligament was in complete chronic tear and that my meniscus joint is about to crack. This kind of surgery is uncommon and I was advised to be operated ASAP. The surgery will take place in less than 10 days from now.

I have also been told to prepare for 2 months medical leave as I will have to undergo a rigorous theraphy exercise for me to walk again.

I hereby request the company to consider the long period of medical leaves and that all cost of my surgery and hospitalization to be borne by the group insurance. Early impression I got from the doctor is that it will cost me approximately around RM20K++. It will, however can be confirmed next week after my further appointment with the doctor.

I hope my requests are given due considerations and look forward for your reply ASAP.

Wish me luck kids. This is difficult for me to have me leg be operated and having emotional breakdown at the same time. I appreciate any support I got from anyone. :(
